Eligibility for Services

0 - 3 years old: Referrals for Early Intervention services are made through the Knox County Health Department's Help Me Grow program. Eligibility and plans for services are created in partnership with Knox County Board of Developmental Disabilities Early Intervention specialists.

To see if your child is eligible for services contact Help Me Grow at 740-392-2200 or 1-800-755-GROW

Age 3 and older: If you are interested in receiving supports through the Knox County Board DD you may:

  1. Come into the office and pick up the application information.
  2. Call the Intake Department at 740-397-4607 and the information will be mailed to you.
  3. Review, print and complete forms that are located on this website and mail or bring the forms to the agency.

If you need help, the intake person will explain the steps to take to apply for enrollment and will assist you with completing the application and getting the documents you may need.

We will need:
• A completed application
• Documentation from a licensed professional (physician, psychiatrist, psychologist) stating your diagnosis & the date of onset. Documentation examples are a psychological evaluation, physical examination report, school IEP & MFE, or a letter from your physician.

Eligibility may be re-determined periodically to ensure that an individual is still eligible for services.

The Family Support Group is open to anyone who is a family member, friend, guardian, or natural support, to an individual with a developmental disability. The purpose of the group is to provide support, share resources, receive trainings on special topics, provide input and direction to the Board of Developmental Disabilities, and receive updates on Developmental Disabilities events.

All meetings take place at the Board of Developmental Disabilities Administrative offices, 11700 Upper Gilchrist Road, Mount Vernon. For more information, please contact Molly Wobbecke at 740-397-4621.

Parent Mentor Group

The Knox County Board of Developmental Disabilities is sponsoring the formation of a parent mentoring group. We are in the process of recruiting parents interested in sharing their experience and receiving specialized training to provide
support and guidance to other parents of school aged
children with disabilities. Parents, with the support of the Ohio Coalition for the Education of Children with Disabilities and the Knox County Board of Developmental Disabilities, will have access to a
Parent Mentor network. For more information contact Jeffrey Williams at 740-397-4647
